0165(2)(d) in the toddler room and outdoor area, there were some plastic bags accessible - assistant director Garcia removed during the on-site visit on 4/11/2023. 0165(3)(f) in the toddler room - a shelf - fine motor activities needs to be secure to the wall to prevent from falling to child care children. 0165(3)(g) in the infant room, preschool room and outdoor the following items were in poor condition or flaking paint: bottom of the sink in the infant room had paddle of water, water heater is not working - water temperature 53 F, preschool room - front window has a gap needs to be sealed, outdoor a white shelf is in poor conditions, tarp used for covering the sand is in poor condition, picnic tables and benches have flaking paint, children playhouse in the playground needs to be re-paint, outdoor - slide needs to be repaired - exposing screws and broken wood in the stairs and begining of the slide.
